Insurance and Payment Information
The Women's Centre for Well Being is a provider for Medicare and most major insurance plans, and provides insurance billing. You are responsible for co-payments or charges that are not covered by your insurance. If you have questions regarding billing or which insurance plans we accept, please contact us. Questions regarding insurance coverage and benefits should be directed to your employer or insurance company.
We expect payment at the time of service either in full or as designated by your insurance plan. As convenience for you, we accept cash, check, American Express, Discover, MasterCard, Visa or Debit Cards as payment options.
Our office hours for appointments are 8:00 a.m. until 5:00 p.m., Monday through Thursday and on Friday from 9:00 a.m. until 12:00 noon. Please tell the receptionist if you need to be seen promptly due to a problem.
If you are unable to keep an appointment, please call as soon as possible so your appointment can be rescheduled and your time may be used for another patient.
All of your medical records are confidential, and no information is given over the telephone without your written consent. Your written authorization is required before your records will be released to insurance companies or others. There is a minimum processing fee of $25.00 for reproduction and release of records.
If you have non-emergency questions, or are calling to get laboratory reports, please call between 8:30 a.m. and 4:30 p.m. Monday through Thursday and on Friday from 9:00 a.m. and 12:00 p.m. when your medical records are readily available.
If you are calling for a prescription refill, please call during the office hours so we can check your records to make sure a refill is the best course of treatment. You also should have the name and telephone number of your pharmacy available before your call.
Our Notice of Privacy Practices describes how medical information about you may be used and disclosed and how you can get access to this information. It is given to all new patients, and a copy is available upon request.
